Marie Kalagayan, BSN, RN, ANCC-CM, is the Founder and Lead Placement Specialist of SCN Placement Experts. With nearly 18 years of healthcare experience since beginning her career in 2008, Marie has built a diverse background in acute care, post-acute care, case management, senior living, and care transitions. She is passionate about helping individuals and families navigate complex healthcare decisions with confidence, compassion, and personalized support.
As an ANCC Board Certified Case Manager, Marie has extensive experience partnering with hospitals, skilled nursing facilities, physicians, home health agencies, hospice providers, and senior living communities to coordinate safe, timely, and appropriate transitions of care. Her expertise includes discharge planning, senior living placement, care coordination, and connecting clients with resources such as private-pay communities, CalAIM Community Supports, and the Assisted Living Waiver (ALW) Program.
Marie founded SCN Placement Experts with the mission of simplifying the placement process while ensuring every client receives individualized guidance. She carefully evaluates each person’s medical needs, functional status, financial considerations, lifestyle preferences, and family goals to identify the most appropriate care setting.
